About CUS files
The .cus file extension is most commonly used to store character customization data for MMORPGs like Lost Ark and Elsword. Players export these files to save their avatar's appearance sliders, hairstyles, and facial features. They use these files to share designs online or reuse them for new characters.
These files are specifically meant to be opened directly within the character creation screen of the respective game. However, the extension is also used for entirely different purposes across industries. It serves as a vehicle diagnostic parameter list for Vector CANoe or the Nissan CONSULT system, an AutoCAD custom dictionary, and even as compressed digital X-ray images used by Carestream Health.
The .cus format is strictly proprietary and context-dependent. You cannot open a Lost Ark .cus file in a standard image viewer to see what the character looks like, nor can you easily edit it in a text editor because it consists of encoded slider coordinates. This makes sharing appearances frustrating, as users are forced to load the heavy game client just to preview a design. For the Carestream X-ray files, hospitals often struggle to share patient data because the format requires specific, expensive medical software.
For game files, users typically want to convert them to standard images like JPG or PNG to preview the character. For Carestream .cus files, converting to DICOM or standard image formats is highly requested. However, achieving this is difficult.
This file format is notoriously difficult to open or convert because the data is encoded specifically for the original software's engine. Often, only the original software can properly read or export the data. Standard online converters fail to process it. We can inspect the file and show text or internal content. If our analysis detects a supported underlying or embedded format - such as the 7z archive structure used in Carestream medical files - viewing or extraction may still be possible.

The CUS Converter Story
The history of Convert.Guru began over 25 years ago in California with Tom Simondi’s file-format database. A former contributor to Space Shuttle development and a software pioneer of the 1980s, Simondi established a trusted resource for file type analysis that was even referenced by Microsoft Windows XP. Today, we use modern technology to process and convert thousands of file formats while continually improving our CUS converter.
